The ENT emergency house-call service is a private medical first-line care option that examines and treats acute ear, nose and throat complaints at home anywhere in Berlin outside practice hours, daily from 6 to 24 hours, with a usual arrival time of 60 to 90 minutes. Acute ENT complaints often occur in the evenings, at weekends or on public holidays, precisely when your own ENT practice is closed. In many cases an initial general medical assessment is enough to classify the complaint, relieve acute pain and plan appropriate further care. A private medical house call is a calm alternative to a crowded emergency clinic. Typical ENT Complaints Outside Surgery Hours

The most common reasons for calling an emergency medical service with ENT-related issues are sudden ear pain, often in the context of a middle ear infection (otitis media). In children this is a particularly typical nocturnal occurrence. Equally common are acute sore throats with difficulty swallowing and fever, as occur with tonsillitis and pharyngitis. Sinus complaints with head pressure, fever and impaired nasal breathing (an acute sinusitis) are often experienced as burdensome only at the weekend. Added to this are sudden hoarseness, a temporary loss of voice with laryngitis, severe dry cough, one-sided hearing loss, a feeling of pressure in the ear following a cold, and sensations of a foreign object in the throat. For most of these situations immediate specialist ENT care is not necessary, but a timely medical assessment with initiation of therapy within a few hours certainly is.

When a Medical Examination Is Advisable at the Weekend

A medical assessment is particularly advisable when complaints are severe or worsen rapidly. Concrete thresholds are a high fever above 39 degrees, intense one-sided ear pain, pronounced difficulty swallowing with markedly reduced fluid intake, and symptoms that persist or increase over several days despite self-treatment. Because many ENT practices are closed on Saturdays and Sundays, the question of rapid care arises regularly on these two days. A physician can assess on site whether medication is sufficient, whether a bacterial infection is present, and whether a follow-up specialist examination is required the next day, and thereby help prevent an initially harmless infection from developing into a complication such as mastoiditis or a peritonsillar abscess.

When a General Private Physician Helps, and When the Specialist Is Needed

A general practitioner can examine, classify and in many cases immediately treat the most common acute ENT complaints. This includes otoscopy to assess the external auditory canal and eardrum, inspection of the throat and tonsils, palpation of the cervical lymph nodes, pressure examination of the four sinuses, and general clinical examination. Pain medication or, when clearly indicated, an antibiotic can be prescribed directly on site.

Certain findings, however, require specialist ENT diagnostics. These include suspected sudden sensorineural hearing loss, unexplained persistent tinnitus, eardrum perforation, microscopic assessment of the auditory canal, and endoscopic examination of the nose and larynx. For these matters the ENT on-call service of the Association of Statutory Health Insurance Physicians, an ENT specialist the following day, or a hospital clinic is the right option. The private medical house call positions itself in between: as rapid initial care, assessment and treatment initiation within 60 to 90 minutes, not as a substitute for instrument-based specialist diagnostics.

The House Call by a General Practitioner, What Happens On Site

The physician arrives with mobile equipment sufficient for the most common acute ENT questions: an otoscope, spatula, headlamp, thermometer, stethoscope, an emergency kit, and prescription forms and certificates for the certificate of incapacity for work. The conversation covers disease onset, progression and accompanying symptoms such as fever, ear drainage, nausea or dizziness. The clinical examination then follows, which in most cases produces a clear treatment recommendation, from pain relief and decongestant nasal sprays through lozenges to antibiotics for bacterial infections. The examination usually takes twenty to thirty minutes.

ENT Emergencies Where 112 or the Hospital Emergency Department Is the Right Choice

Certain ENT symptoms indicate a situation where immediate hospital care is necessary and a house call would not be the appropriate setting. Severe shortness of breath with progressive airway constriction, pronounced swelling of the tongue, throat or neck, sudden one-sided hearing loss with vertigo, severe uncontrollable nosebleeds (epistaxis), and ENT injuries following trauma belong in the emergency chain via emergency call 112 or in the emergency department of a hospital with ENT availability. Sudden hearing loss with suspected sudden sensorineural hearing loss should also be evaluated by a specialist within a few hours. In Berlin the ENT emergency departments of major hospitals such as the Charité and several Vivantes locations are available around the clock for this purpose.

The Care Tiers Outside Practice Hours at a Glance

The German healthcare system is divided into clearly delineated care tiers, including at the weekend. General and specialist practices provide regular care during normal opening hours. The medical on-call service of the Association of Statutory Health Insurance Physicians (reachable nationwide on 116117) assesses acute but non-life-threatening complaints outside these hours and runs on-call clinics. For life-threatening emergencies the rescue service is responsible via 112. The private medical house call complements this structure as a fourth option: timely care at home within 60 to 90 minutes, without a waiting room and without travelling to an emergency practice, useful above all when a practice visit is made difficult by fever, pain or a strong feeling of illness.

Procedure and Arrival

The procedure begins with a telephone call to 030 550 77 870, in which the complaint, previous illnesses and any self-treatment to date are discussed. On this basis it is decided whether a house call is the appropriate option. The usual arrival time is 60 to 90 minutes. No special preparation is required; helpful items are the insurance card, the vaccination record if applicable, and a list of regularly taken medications. Prescriptions and a certificate of incapacity for work are issued directly on site if clinically indicated. Frequently Asked Questions About the ENT Emergency Service

Will I receive antibiotics during the house call?

When clearly indicated (for example in bacterial tonsillitis, purulent middle ear infection or bacterial sinusitis) an antibiotic can be prescribed directly on site. For viral infections, which account for most acute ENT complaints, symptomatic treatment is the focus; the physician decides after the examination.
